Description

• Conduct assessments of patients in both inpatient and outpatient environments and formulate customized treatment plans
• Implement therapeutic interventions with a focus on enhancing mobility, strength, balance, and overall function
• Work closely with nurses, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes
• Provide educational support to patients and their families regarding therapeutic objectives, discharge procedures, and home exercise regimens
• Document patient evaluations, treatments, and progress in alignment with organizational and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure continuity of care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Active state license as a Physical Therapist
• CPR certification preferred
• Specialized certifications (neurological, geriatric, etc.) are advantageous but not mandatory

Skills

• Previous experience is highly desirable
• Proficient in treating patients with diverse and complex medical issues
