Care Initiatives is currently seeking a PRN Visit Nurse to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities.

What It Takes To Be A RN Hospice Visit Nurse

• Collaborate with the physician to oversee and implement personalized care plans, ensuring the active involvement of interdisciplinary teams to achieve the optimal outcomes for each patient
• Provide consistent, high-quality patient care to ensure an exceptional and seamless patient experience
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures
• Provides high-quality patient care while ensuring clear, concise, and accurate clinical documentation
• Assesses the need for plan of care updates while keeping the patient and family involved in care planning decisions

What It Takes To Become A Hospice Visit Nurse:

• RN with a minimum of two years of recent clinical nursing experience.
• Current, valid CPR certification.
• Demonstrated patience and a supportive and sensitive approach to providing patient, family, and caregiver support.
• Willingness and ability to provide care in facility and patient home settings.
