Key Responsibilities\n

• Conduct therapeutic sessions remotely, using video conferencing tools to connect with clients and their families in a safe and supportive manner.

• Collaborate with prescribing clinicians to develop integrated treatment plans that address both medication management and therapeutic interventions.

• Regularly evaluate and monitor client outcomes through established protocols, ensuring the highest level of care.

• Engage in continuity of care and discharge planning, facilitating advanced care planning discussions with clients and families.

• Design individualized treatment plans tailored to each client’s psychiatric diagnosis, presenting problems, and treatment goals, adhering to best practices in social work.

• Document all patient care activities in our electronic health record system, following departmental guidelines to maintain compliance and quality.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related field is required.

Compensation and Benefits\n
