As part of our specialized Hospice Center, you will work collaboratively with a dedicated team of professionals focused on providing compassionate care. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children to identify developmental delays and create tailored intervention strategies.

• Play an active role in the development and implementation of Individualized Family Service Plans (IFSP), ensuring alignment with family goals and child development milestones.

• Monitor and evaluate progress as part of a multidisciplinary team while maintaining compliance with all federal and state regulations.
\n
• Utilize evidence-based practices and Natural Learning Environment Practices to foster a supportive and engaging atmosphere for children and families.

• Provide ongoing education and resources to families that enhance their child's development, focusing on practical strategies that can be implemented in daily routines.

• Build trusting relationships with families, cultivating an environment where they feel comfortable expressing their concerns and priorities.

• Adapt your approach to meet the diverse needs of individual children and families, incorporating various learning styles in your intervention techniques.

Patient Population / Client Focus\n

Your primary focus will be on infants and toddlers experiencing disabilities or developmental delays. The children you support will range from birth to three years old, and your contributions will directly impact their early developmental outcomes. You will engage with families from diverse backgrounds to empower them in advocating for their child's growth and developmental journey.

Qualifications\n

• A Master's Degree or Doctorate in Social Work, with an active LCSW license from the Department of Public Health.
