Empower Well Therapy is actively seeking a full-time Licensed Clinical Social Worker (LCSW) to join our dedicated team in Buffalo, New York. This remote position is ideal for an experienced practitioner passionate about supporting young children and their families through various challenges associated with developmental delays and disabilities. As an LCSW, you will play a critical role in assessing, planning, and delivering individualized interventions to promote the healthy development of children in their early years.

Work Environment\n

This position operates in a remote environment, allowing for flexibility while ensuring that you can effectively support the families and children you work with. Our Hospice Center provides essential resources and support to aid our social workers in delivering comprehensive care. While this role is primarily remote, you will collaborate closely with a multidisciplinary team and engage with families through virtual consultations.

Key Responsibilities\n

• Conduct thorough evaluations and assessments of children with developmental delays or disabilities to inform tailored care plans.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs).
\n
• Monitor individual outcomes and progress as part of a collaborative team, adjusting interventions as necessary.
\n
• Offer supportive resources and strategies to families, promoting their child’s development in various domains, including social, emotional, and cognitive skills.
\n
• Work in compliance with all applicable Federal and State regulations, ensuring ethical and high-standard practices.
\n
• Establish trustful relationships with children, families, and team members to create a safe and effective learning environment.
\n
• Adapt interventions based on the diverse learning styles of children, employing evidence-based practices and Natural Learning Environment principles.
\n
• Maintain a schedule that reflects the frequency of services required based on development assessments and family needs.
\n
• Utilize natural environment teaching and coaching models during family interactions to enhance learning opportunities.
\n
• Adhere to essential health and safety standards to safeguard both staff and individuals receiving care.

Patient Population / Client Focus\n

Your primary focus will be on young children, specifically infants and toddlers facing unique development challenges. Our Children’s Services program involves close collaborations with families to facilitate growth and learning through holistic approaches tailored to their individual circumstances. You will connect with families on a personal level, helping them navigate resources and creating strategies tailored to each child's developmental needs.

Qualifications\n

• A Master's Degree or Doctorate in Social Work from an accredited institution.

• Current LCSW licensure recognized by the Department of Public Health.

• A valid driver’s license is required to facilitate occasional travel for client interactions.
\n
• Advanced skills in communicating effectively with diverse populations and adapting strategies to fit individual learning and development styles.

• Proven commitment to principles of Natural Learning Environment practices.
\n
• Proficiency in the coaching model and natural environment teaching methodologies.
\n
• Strong organizational and time management skills to manage caseload effectively.
\n
• Collaborative spirit with excellent interpersonal skills to foster teamwork.

Preferred Experience\n
