We are seeking a dedicated and passionate Licensed Clinical Social Worker (LCSW) to join our team at the Niagara Falls Health & Wellness Facility in Chino, California. This full-time, remote position is integral to our pediatric hospice services, where you will provide compassionate care, conduct assessments, and participate in interdisciplinary efforts to support families during critical times. We proudly offer a competitive salary range of $75,000 – $80,000 annually.

Work Environment\n

This role is set within our Hospice Center, allowing you to work remotely while also engaging with families and children in need of critical support. Key Responsibilities\n

As an LCSW, you will perform a variety of essential functions including:

• Conducting detailed evaluations and assessments to determine the needs of children and their families.

• Collaborating with multidisciplinary teams to develop and implement Individualized Family Service Plans (IFSP).
\n
• Monitoring progress and outcomes while adjusting interventions as necessary to meet each family's unique needs.

• Providing direct support and evidence-based strategies to families to enhance their understanding of their child's developmental progress.

• Utilizing coaching models and natural learning environment practices to facilitate learning and growth.
\n
• Building rapport with children, families, and team members to cultivate a supportive and productive environment.

Patient Population / Client Focus\n

Your primary focus will be on infants and toddlers experiencing disabilities or developmental delays. You will work directly with families in our pediatric hospice care program, offering your expertise and empathy to help them navigate medical, emotional, and social challenges. Through a combination of in-home support and community resource advocacy, you will make a profound impact on the lives of both children and their families.

Qualifications\n

Applicants must meet the following qualifications:

• Master's degree or Doctorate in Social Work.

• Current licensure by the Department of Public Health as a Licensed Clinical Social Worker (LCSW).
