Key Responsibilities\n

• Conduct thorough clinical assessments to determine patient needs and develop customized treatment plans.

• Lead psychoeducational and process-oriented therapy groups, providing guidance and support tailored to individual and collective needs.

• Begin discharge planning on the day of patient admission, ensuring patients and families are well-informed about transition processes.

• Facilitate discharge sessions, confirming that coping strategies, transition plans, and available resources are clearly understood by patients and loved ones.

• Coordinate communication with referral sources and outpatient providers for seamless continuity of care across all stages of treatment.
\n
• Facilitate patient transfers to higher levels of care when necessary, prioritizing patient well-being and safety.
\n
• Actively participate in clinical outreach initiatives, representing our organization in various community contexts.
\n
• Manage crises effectively during therapy sessions and in the milieu, ensuring a safe environment for all patients.
\n
• Support milieu staff in interactions with patients and families, reinforcing therapeutic goals and enhancing emotional support.
\n
• Collaborate with the multidisciplinary treatment team to uphold the highest levels of clinical quality and patient outcomes.
\n
• Conduct risk assessments for suicidality and potential harm to self or others, taking necessary actions to ensure patient safety.

• Provide support to other clinical team members in their absence, ensuring continuity of care.
\n
• Communicate with the Utilization Review Department to address authorizations and billing processes effectively.

Patient Population / Client Focus\n

This role focuses primarily on hospice patients, many of whom are experiencing significant emotional and psychological challenges. You will work closely with individuals and families during one of the most sensitive times in their lives, providing support to manage grief, loss, and transitions. Qualifications\n
