Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand clients’ mental health challenges, strengths, and support systems, helping to identify areas for intervention.

• Deliver individual, family, and group therapy using evidence-based modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care.
\n
• Assemble and implement personalized treatment plans that address specific client needs and goals, fostering engagement and commitment to the healing process.

• Work in tandem with psychiatrists, psychologists, and other healthcare providers to ensure a holistic and coordinated approach to each client’s care.
\n
• Utilize crisis intervention techniques, perform risk assessments, and create safety plans when necessary to support clients through acute mental health challenges.

• Maintain clear and accurate documentation of client interactions, treatment progress, and assessments in accordance with legal and ethical standards.

• Facilitate and lead group therapy sessions, psychoeducation workshops, and support groups to build community and resilience among clients.

Patient Population / Client Focus\n

This role primarily focuses on serving individuals in hospice care, many of whom may be coping with grief, anxiety, depression, and complicated emotions associated with life-limiting diagnoses. You will have the profound responsibility to provide empathy and understanding, creating a safe space for patients and their families during one of the most challenging periods of their lives.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• An active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in a mental health setting, ideally in outpatient care or hospice.

• Sound knowledge of therapeutic modalities and treatment methods, coupled with excellent interpersonal and communication skills.

Compensation and Benefits\n
