Key Responsibilities\n

• Conduct thorough and age-appropriate biopsychosocial assessments to evaluate mental health, substance abuse, and medical needs, ensuring a holistic understanding of each patient's situation.

• Deliver evidence-based therapeutic services, engaging individuals, couples, and families in a variety of modalities including counseling, crisis intervention, and treatment planning.

• Document and maintain accurate electronic health records, including progress notes, mental health evaluations, and suicide risk assessments, while remaining compliant with HIPAA regulations.

• Establish vital connections for patients by providing referrals to local resources and community agencies, including medication assistance programs and support groups.

• Collaborate with Quality Improvement teams to innovate and improve data collection processes, contributing to organizational initiatives aimed at enhancing service delivery and patient outcomes.

Patient Population / Client Focus\n

This position primarily serves patients in hospice care, many of whom face end-of-life challenges and require sensitive and competent mental health support. Qualifications\n

• A valid and up-to-date state license as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
