Key Responsibilities\n

• Initiate discharge planning on the day of patient admission, ensuring a proactive approach to patient care.

• Conduct discharge sessions with patients to discuss their coping strategies, transition plans, and available resources, ensuring they leave well-prepared.

• Confirm discharge appointments with patients and their families, guiding them through the next steps in their care journey.
\n
• Maintain open communication with referral sources and outpatient providers throughout the patient’s treatment for consistent continuity of care.
\n
• Facilitate patient transfers to higher levels of care as necessary, advocating for the best interests of our patients.
\n
• Actively engage in clinical outreach efforts and program tours, representing our services to the community effectively.
\n
• Manage crises that arise during sessions or in the care setting with professionalism and expertise.
\n
• Provide guidance and support to milieu staff to enhance their interactions with patients and their families, fostering a supportive environment.
\n
• Collaborate with the entire treatment team, ensuring that every aspect of a patient’s care is coordinated and of the highest quality.
\n
• Assess patients for potential suicidality and risk, conducting thorough evaluations to safeguard their well-being.
\n
• Offer clinical support to fellow team members, stepping in when additional assistance is required.
\n
• Collaborate with the Utilization Review Department to ensure that patient care is authorized and aligned with business department guidelines.

You will work closely with individuals and families facing the challenges of serious illnesses, providing empathetic support and tailored interventions. Qualifications\n

• A master's degree in Social Work, Counseling, Psychology, or another related mental health discipline.

• Current and independent licensure as a Licensed Clinical Social Worker (LCSW).
