• Triage and Management of Clinical Caseload: Identify high-risk situations within your patient population, ensuring a balanced caseload that addresses both urgent referrals and ongoing care needs. Collaborate actively with case managers to streamline discharge planning and enhance the continuum of care.

• Legal and Ethical Standards: Uphold the highest ethical standards in all assessments and interventions, adhering to policies regarding abuse reporting, patient rights, and confidentiality.

• Documentation: Maintain accurate and up-to-date records, including interdisciplinary care plans, educational needs assessments, and progress notes, in compliance with organizational policies.

• In-Service Training: Provide educational sessions for staff on psychosocial aspects of patient care, such as Lunch and Learn presentations, to enhance team understanding of patient needs.

• Additional Responsibilities: Engage in hospital activities such as Social Work Month, support groups, and educational programs for patients, families, staff, and volunteers.

Qualifications\n

Compensation and Benefits\n
