Key Responsibilities\n

• Conduct thorough assessments of clients over the phone or via video conferencing to gather critical information relevant to their psychological and emotional needs.

• Engage clients in meaningful therapeutic conversations to identify beneficial activities and support systems tailored to their individual situations.

• Provide timely crisis intervention to clients experiencing significant distress, employing evidence-based strategies to stabilize their emotional state.

• Develop and implement personalized treatment plans that align with clients' healthcare needs and preferences.

• Maintain accurate and confidential documentation of client interactions and treatment progress, preparing detailed reports for internal and external audits as necessary.

• Connect clients and their families with appropriate community resources and services, ensuring they have access to the support they need.

• Participate in regular training, supervision, meetings, as well as peer supports to enhance clinical practice and improve service delivery.

• Recognize signs of child abuse or neglect in family dynamics, making appropriate referrals to ensure client safety and well-being.

Patient Population / Client Focus\n

Your primary focus will be on clients addressing the complexities surrounding hospice care, including emotional fallout from terminal diagnoses, caregivers' needs, and bereavement. Qualifications\n

• A Master's degree in Social Work (MSW) or a closely related field from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Connecticut.

• Proficient in electronic record-keeping and familiarization with various software tools (such as Microsoft Word, Excel, Outlook, and PowerPoint).

Compensation and Benefits\n
