Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observations, and comprehensive reviews of medical and treatment records. Evaluate how emotional, social, and environmental factors affect our patients’ recovery journeys.

• Deliberately provide emotional support and counseling to patients and their families, addressing adjustment issues, coping strategies, and promoting mental well-being.

• Collaborate closely with an interdisciplinary rehabilitation team to craft individualized treatment plans that take into account the patients’ unique psychosocial needs.

• Assist patients in identifying and accessing vital community resources, such as support groups, vocational rehabilitation services, and housing assistance.

• Deliver education and ongoing support to families regarding available community resources and post-rehabilitation care options.

• Actively participate in discharge planning meetings, ensuring a seamless transition in care, and representing the social work department's recommendations.

Patient Population / Client Focus\n

Your primary clientele will include patients within a Hospice capacity who are facing end-of-life challenges, as well as their families. You will engage with diverse cultural backgrounds, necessitating culturally sensitive approaches to care and support. Each interaction will aim to enhance the patients' emotional and psychological well-being during a pivotal life stage, aiding them in finding peace and acceptance.

Qualifications\n

• Must hold a current and valid Licensed Clinical Social Worker (LCSW) credential.
