Key Responsibilities\n

• Conduct thorough psychosocial assessments through detailed interviews and careful analysis of medical records to evaluate emotional, social, and environmental factors impacting patient recovery.

• Provide compassionate counseling and emotional support to patients and their families, enabling them to successfully adjust and cope with their situations.

• Collaborate closely with the rehabilitation team, contributing insights to develop individualized treatment plans that reflect each patient’s specific psychosocial needs and rehabilitation goals.

• Assist patients in navigating and accessing community resources, including support groups, vocational rehabilitation services, and housing assistance, tailored to their unique circumstances.

• Educate and inform patients and families about available community resources and post-rehabilitation services, ensuring they are well-prepared for discharge.

• Participate actively in discharge planning, facilitating smooth transitions for patients returning home or shifting to other care settings.

• Utilize electronic health record (EHR) systems for documentation and patient management, maintaining accurate and confidential patient information according to best practices.
