Key Responsibilities\n

• Psychosocial Assessment: Conduct thorough assessments that consider individual and family dynamics as well as societal factors impacting treatment outcomes. Utilize your advanced understanding of human behavior to identify key psychosocial factors that may affect patients.

• Clinical Interventions: Provide supportive counseling for students and their families, facilitate the processing of difficult decisions—such as end-of-life care or placement options—and link families with necessary community resources. Coordinate with case managers to optimize discharge planning.

• Triage and Management: Effectively juggle a diverse caseload, balancing referrals, high-risk admissions, and ongoing support needs, while recognizing when to seek assistance from colleagues or supervisors.

• Legal and Ethical Standards: Uphold California's legal requirements and ethical guidelines in all assessments and interventions. Adhere strictly to Tenet Policies and Procedures within Social Services.

• Documentation: Ensure meticulous documentation of all activities, including the Interdisciplinary Plan of Care and Social Services Progress Notes, compliant with departmental policies.

• In-Service Training: Provide educational sessions to other hospital staff about psychosocial issues affecting patient care, including conducting 'Lunch and Learns' to share insights and promote awareness.

• Quality Improvement: Actively participate in the analysis and improvement of patient care metrics, contributing to enhanced service delivery and patient outcomes.

• Other Duties: Engage in additional responsibilities such as community outreach programs and support groups dedicated to cancer patients and their families.

Compensation and Benefits\n

The annual salary range for this position is between $120,000 to $130,000 , commensurate with experience and qualifications.
