Work Environment

This role is primarily onsite, delivering services to patients within their homes as part of our Home Care facility in Albany, NY. You will be part of a collaborative and supportive work environment that values teamwork and effective communication. Our therapists have access to advanced therapeutic equipment and electronic medical record systems to efficiently document client progress and treatment plans.

Key Responsibilities

• Conduct thorough physical therapy evaluations for a diverse range of patients, including children and adults, assessing their physical abilities, limitations, and specific needs.
• Develop and execute individualized treatment plans based on evaluation findings and patient goals, applying evidence-based techniques.
• Regularly reassess patient clinical signs and symptoms to monitor treatment effectiveness and adjust goals and methodologies as required.
• Document all patient interactions meticulously using the company's electronic record system, including SOAP notes and progress summaries.
• Provide education to patients and their families regarding the recovery process, potential challenges, and appropriate home exercises.
• Meet established service utilization goals while actively contributing to the business's overall objectives.
• Collaborate with other healthcare professionals within Ensemble Therapy to ensure comprehensive and integrated care for clients receiving multiple services.

Patient Population / Client Focus

Our clientele is diverse, ranging from children to adults with orthopedic conditions requiring rehabilitation. You will engage with patients with varying levels of mobility and functional independence, from post-operative recovery to chronic pain management. The focus is on promoting independence and improving quality of life through tailored rehabilitation programs.

Qualifications

• A Doctor of Physical Therapy (DPT) degree from an accredited program is required.
• Current and unrestricted state licensure to practice physical therapy in New York.
• Willingness to provide evaluations and treatments both in-person and via telehealth platforms when clinically appropriate.
• A strong desire to serve a wide-ranging patient demographic, including varied age groups and cultural backgrounds.
• Flexibility to adapt to a rapidly changing healthcare environment and patient needs.
• Excellent teamwork, collaboration skills, and a commitment to exceptional customer service.

Preferred Experience

• Previous experience working in a home care setting or with orthopedic patients is highly desirable.
• Familiarity with electronic health records and theraguns is a plus, showcasing proficiency in modern therapeutic tools.
• Experience crafting individualized care plans that incorporate multidisciplinary perspectives.

Compensation and Benefits
