Key Responsibilities\n

• Engage with patients and their families to provide emotional support and encouragement during behavioral health crises.

• Conduct comprehensive assessments to identify resourcing needs and care gaps, actively engaging in case review processes.

• Facilitate psychoeducation sessions for patients and families, assisting them in setting and achieving individual recovery goals.
\n
• Evaluate social determinants of health, addressing barriers to care such as housing insecurity, financial constraints, and transportation issues.
\n
• Establish connections with support groups and community resources to promote mental wellness and recovery.
\n
• Collaborate with an interdisciplinary team to ensure treatment plans are aligned with the unique needs of each patient and family.

• Participate in scheduled consultations to review and monitor patient progress regularly, ensuring continuity and quality of care.

Patient Population / Client Focus\n

Your primary focus will be on families impacted by acute behavioral health issues, including but not limited to suicidality, severe persistent mental illness, and psychiatric emergencies. Your experience with a diverse population of children, adolescents, and adults will be invaluable, as you guide these families through challenging times, providing hope and support in their recovery journeys.

Qualifications\n

• A Master's degree in Social Work (MSW) is required to ensure a comprehensive understanding of clinical practice and methodologies.

• Possession of an active and valid license to practice social work in the state of California.

• Proficiency in managing cases involving children and adolescents in both outpatient settings and through telehealth platforms.

Compensation and Benefits\n

Your expertise will be rewarded with a competitive salary ranging from $110,000 to $130,000 per year, supplemented by a comprehensive benefits package.
