Licensed Master's Social Worker for Community Health and Senior Care

Position Overview

Mindful Steps Counseling Services is seeking a dedicated and compassionate Licensed Master's Social Worker to join our dynamic team in Morristown, New Jersey. Work Environment

This is a fully remote position, allowing for flexibility and a balance between work and personal life. While most interactions will occur virtually, you will be expected to engage in regular visits to clients’ homes or care facilities to monitor their conditions and address their needs. Scheduled work hours are from Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client availability and urgent situations. Flexibility in scheduling is necessary to ensure continuity of care and meet the demands of your clients.

Key Responsibilities

• Perform comprehensive assessments of seniors’ psychosocial needs by examining emotional, mental, and social factors influencing their well-being.
• Develop and implement personalized care plans in collaboration with clients and their families, specifically addressing their goals and preferences.
• Provide counseling and emotional support to seniors and their families, assisting them in navigating transitions such as relocation to assisted living or coping with health changes.
• Act as an advocate for clients’ rights, ensuring they have access to necessary resources and services within the community.
• Coordinate with interdisciplinary teams including healthcare professionals, caregivers, and nurses to create an integrated care approach tailored to individual client needs.
• Conduct regular check-ins with clients, monitoring their well-being, living conditions, and addressing any safety concerns that arise.
• Facilitate communication and mediate conflicts among family members involved in the senior’s care to enhance cooperation and support.
• Educate clients and families about the array of community resources available to them, enhancing their social connections and overall quality of life.
• Maintain meticulous documentation of client interactions, assessments, care plans, and their progress, ensuring compliance with relevant policies and ethical standards.
• Stay up-to-date on best practices in social work, including changes in regulations and resources available for seniors.

Patient Population / Client Focus

Your primary focus will be on seniors, a demographic often faced with complex emotional and social challenges. By addressing their needs holistically, you will help improve their quality of life and foster an environment of dignity and respect. This role will require you to navigate sensitive discussions around end-of-life decisions, mental health crises, and family dynamics, making empathy and strong interpersonal skills essential.

Qualifications

• Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• At least two years of relevant experience in a social work capacity, ideally focused on geriatric care or within a community health setting.
• In-depth understanding of social work principles, geriatric care issues, and familiarity with local community resources.
• Proven skills in assessment, counseling, and conflict resolution.
• Strong communication skills and the ability to work collaboratively with multidisciplinary teams.
• Comfortable using electronic health record (EHR) systems and basic computer applications for documentation.

Preferred Experience

• Experience in working within a community health clinic or similar environment.
• Familiarity with local resources and services available to seniors.
• Demonstrated ability to develop engaging programs or support groups focused on senior issues.

Compensation and Benefits
