(PT and FT)* Malpractice Insurance Provided* PTO & Holiday (Full-Time)Key Responsibilities:* Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.* Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.* Develop and implement evidence-based treatment plans tailored to the needs of each patient.* Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.* Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).* Monitor patient progress and adjust treatment plans as needed.### Requirements* Active and unrestricted license to practice as a Licensed Clinical Social Worker in State applying for.* Effective oral and written communication in English* Basic proficiency with technology, including electronic health records (EHR).* Strong organizational and documentation skills, with attention to regulatory compliance.* Ability to provide in-person services at assigned facilities, up to 45 minutes.* Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.* Full Vaccination and Booster Status may be required in some facilities.### Preferred Qualifications* Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.* History of treating Anxiety, Depression and Adjustment Disorders.* Psych or Addictions experience is a plus.* Excellent time management and organizational skills.SCT welcomes individuals with a variety of licensure types to join our exceptional team.
