Schedule: Full time

Shift: Day (United States of America)

Location: This is a fieldwork position covering Volusia and Flagler Counties.

Provides personal care to patients according to the assignment details and care plan. Documents patient care and observations timely and appropriately. Participates in end-of-life care for patients and families.

Informs the registered nurse of any changes in the patient's condition.

Follows the schedule of care, including making several visits per day with travel.

Assists patients with activities of daily living, such as bathing, dressing, and grooming.

Monitors and records vital signs, including temperature, pulse, and respiration. Supports patients with mobility and exercise as prescribed.

Education:

• High School Grad or Equiv [Required]

Work Experience:

• Experience in the health care field [Required]
• Hospice experience [Preferred]

Additional Information:

• Hospice Aide training from an approved 75 hour training course [Required]

Licenses and Certifications:

• Basic Life Support – CPR Cert (BLS)[Required]
• Driver's License (DL) [Required]
• Certified Nurse Assistant (CNA) [Required]

Pay Range: $15.30 - $22.95
