Patient Care Coordinator

We are seeking a highly skilled, detail-oriented, and compassionate Patient Care Coordinator to join our medical team. This role is responsible for ensuring seamless communication and coordination between patients, healthcare providers, and office staff. The Patient Care Coordinator will serve as a primary point of contact for patients, providing exceptional customer service and helping ensure their healthcare needs are addressed efficiently and effectively.

Responsibilities

• Coordinate and manage patient records, including updates to electronic health records (EHR).
• Utilize computerized systems to manage patient information, including health information management and medical office software.
• Provide support to medical reception staff, including answering patient calls, scheduling appointments, handling insurance-related inquiries, and preparing for patient visits.
• Perform administrative tasks such as data entry, filing, scanning, and preparing documents.
• Coordinate and facilitate communication between patients and healthcare providers.
• Maintain accurate and up-to-date records of patient care, including patient intake and follow-up procedures.
• Utilize medical terminology and knowledge of medical procedures to effectively communicate with patients and healthcare providers.
• Ensure patients are aware of appointments, treatment plans, referrals, and any necessary follow-up care.
• Maintain confidentiality and comply with all HIPAA requirements.

Qualifications

• Medical Assistant experience preferred.
• Proficiency in computerized health information management systems and electronic medical records.
• Strong computer and typing skills with a high level of accuracy.
• Familiarity with medical terminology and healthcare workflows.
• Excellent communication and interpersonal skills.
• Ability to manage multiple tasks simultaneously and prioritize responsibilities.
• Compassionate, patient-focused, and professional demeanor.
• Strong organizational skills and attention to detail.
