Key Responsibilities

• Conduct thorough evaluations, including comprehensive assessments and review of patient history, to identify treatment needs and develop individualized care plans.
• Communicate evaluation results and treatment plans clearly to patients and their families, involving them in goal-setting to foster engagement in their recovery.
• Implement treatment protocols proficiently based on evaluations and the latest healthcare guidelines, ensuring high standards of care are met.
• Document all aspects of patient care, including initial evaluations, progress notes, discharge summaries, and follow-up care, to maintain seamless continuity of treatment.
• Consult with colleagues and other healthcare providers as necessary, adapting care plans in response to observed changes in patient conditions.
• Participate in community outreach initiatives and marketing activities to raise awareness about our services and promote access to care.
• Adhere to all regulatory and accreditation standards to support the operational success of the department.
• Pursue ongoing professional development and continuing education opportunities to enhance clinical competencies and deliver best-practice care.
• Facilitate a positive patient experience by being flexible and adaptable in care provision, ensuring that each patient feels supported and valued throughout their treatment journey.

Patient Population / Client Focus

As a Physical Therapist at Pathways, you will work with a diverse patient population, including individuals recovering from orthopedic surgeries, sports injuries, neurological conditions, and chronic pain. Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current state license to practice as a Physical Therapist.
• Basic Life Support (BLS) certification from the American Heart Association is required.

• A minimum of 2 years of experience in a clinical setting, preferably in outpatient rehabilitation.
• Experience with electronic medical records (EMR) systems and familiarity with current rehabilitation practices is a plus.

Compensation and Benefits
