Work Environment

The work setting is fully onsite in our state-of-the-art rehabilitation facility in Port Charlotte. Our clinic features a warm and inviting atmosphere designed to encourage patient engagement and recovery. The environment promotes teamwork among a diverse group of professionals, including physicians, occupational therapists, and administrative staff. The team operates on a Monday through Friday schedule from 8 AM to 5 PM, allowing for a work-life balance while providing dedicated support to our patients.

Key Responsibilities

• Conduct thorough evaluations of patients' conditions, reviewing pertinent medical histories and utilizing a range of specific assessments.

• Develop and implement personalized treatment protocols while ensuring adherence to evidence-based practices tailored to each patient's specific needs.

• Monitor and document changes in patient conditions, adjusting treatment plans as necessary based on ongoing assessments and collaborative discussions with other healthcare professionals.

• Maintain clear and comprehensive documentation of patient care activities, including initial evaluations, daily encounter notes, progress summaries, and discharge evaluations, ensuring compliance with state and federal regulations.

• Engage actively in discharge planning, setting realistic goals for patients' post-therapy progression and ensuring continuity of care through follow-up communications.

• Participate in facility meetings and training sessions to remain updated on corporate and clinical guidelines, adhering to compliance standards.

• Maintain respectful, open lines of communication with patients, family members, healthcare providers, and third-party payors throughout the therapeutic process.

Patient Population / Client Focus

The primary focus of this role is on orthopedic populations requiring physical therapy services due to surgery, injury, chronic pain, or other musculoskeletal disorders. You will work closely with a diverse range of patients, from athletes recovering from sports injuries to seniors seeking rehabilitation after falls or surgeries. Your role will be critical in implementing tailored therapeutic interventions that optimize each patient's functional independence and overall health.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required to ensure advanced knowledge in patient care principles.

• A valid state license to practice as a physical therapist is required, along with certifications as mandated by state regulations.

• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

Applicants should ideally possess a minimum of two years of relevant experience in a clinical setting, specifically in outpatient orthopedic rehabilitation. Familiarity with various physical therapy modalities, treatment equipment, and interdisciplinary collaboration will be considered advantageous.

Compensation and Benefits
