Genesis Medical Health is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its growing medical team providing primary care to patients within skilled nursing facilities in the Endicott, NY area.

This is a full-time opportunity for a provider who enjoys working independently, building relationships with patients and facility staff, and managing the ongoing medical needs of a geriatric and post-acute population.

Position Highlights

• Full-time, weekday schedule
• Approximately 2530 patients per day
• $130,000 $150,000 base salary

Responsibilities

• Provide primary, post-acute, and long-term care to skilled nursing facility residents
• Perform admissions, follow-up visits, annual assessments, and chronic disease management
• Evaluate changes in condition and address acute medical concerns
• Review labs, imaging, medications, and clinical reports
• Collaborate closely with physicians, nursing staff, specialists, families, and facility leadership
• Help prevent unnecessary hospitalizations through proactive patient management
• Complete documentation and EMR charting in a timely manner

Requirements

• Active New York NP or PA license
• Ability to practice independently and manage a patient caseload
• Strong clinical judgment and communication skills
• SNF, nursing home, primary care, internal medicine, or geriatric experience preferred
• Comfortable caring for medically complex patients
• Reliable transportation
