Key Responsibilities\n

• Conduct comprehensive assessments and develop individualized treatment plans tailored to meet the unique needs of each patient. This includes addressing pre-crisis and crisis planning, risk assessments, and medication treatment plans.

• Utilize evidence-based assessment tools collaboratively with the clinical team to ensure effective diagnosis and treatment strategies.

• Collaborate with interdisciplinary teams to create and implement positive behavioral support plans, enhancing patient engagement and success.

• Actively contribute to licensing and accreditation initiatives, ensuring that Empowerment Pathways upholds the highest standards of care.

• Serve as an essential liaison to various service systems, state agencies, and community partners, facilitating access to additional resources for our clients.

• Maintain open lines of communication that align with contract standards to advance Empowerment Pathways' mission throughout the community.

• Network with local, state, federal, and private organizations to foster partnerships and explore new funding sources for our programs.

• Engage in ongoing professional development activities, including attending conferences, presenting to groups, and participating in task forces that relate to mental health.

• Perform additional duties as assigned by the Clinic Director that contribute to the overall efficacy of our mental health services.
