Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active California Physical Therapist license required to start the assignment.
• BLS certification from American Heart Association or American Red Cross may be required.
• Home Health experience required. Experience conducting skilled home assessments, documenting skilled notes, and providing skilled interventions in patient homes preferred.
• Experience managing caseload productivity expectations in home health settings preferred. Ability to meet point or visit-based productivity targets may be required by client.
• Ability to travel within assigned county territories, including Fremont, Castro Valley, Hayward, Newark, San Leandro, Union City, Oakland, Berkeley, Alameda, and nearby cities. Additional travel outside assigned area may be required.
• Weekend coverage may be required. Candidate must be willing to accept assigned patients and occasional off-area visits to maintain caseload if requested by client.

• Facility Type: Home Health.
• Shift Details: Day shift.
• Assigned Service Areas: Area 1 (Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City) and Area 2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont).
• Weekend coverage and travel outside the assigned service areas may be required.
