Registered Nurse - Hospice

Job Description

Job Summary:

Provide comprehensive skilled nursing care and case management to hospice patients in their homes or at the inpatient Hospice Center. Collaborate with interdisciplinary teams to ensure quality, cost-effective outcomes and deliver compassionate support to patients and families.

Responsibilities:

• Assess, plan, and implement nursing interventions based on systematic evaluations and physician directives.
• Monitor patients' physical and psychosocial conditions, adjusting care plans as needed.
• Oversee and evaluate care provided by Home Health/Hospice Aides and CNAs to ensure adherence to care plans.
• Communicate effectively with healthcare teams, community resources, and support agencies to maintain continuity of care.
• Educate patients and families on symptom management, health promotion, and care options.
• Document patient care accurately and in compliance with agency standards and regulatory requirements.

Qualifications:

• Hold a current RN license in the State of Montana.
• Possess a valid driver's license, auto insurance, and reliable transportation.
• Bring at least 2 years of recent experience in acute care, home care, or hospice settings.
• Demonstrate clinical competencies such as venipuncture, wound care, catheterization, and parenteral therapy.
