Key Responsibilities\n

• Conduct thorough psychosocial assessments via video conferencing, identifying clients' strengths, challenges, and necessary interventions.

• Develop and implement evidence-based therapeutic interventions and treatment plans for individuals, families, and groups suffering from grief, depression, anxiety, and other psychological responses related to hospice care.

• Participate in interdisciplinary team meetings to coordinate integrated care, working closely with physicians, nurses, and other professionals to address clients’ overall wellness.

• Provide advocacy for clients, helping them navigate community resources and support services essential during their hospice journey.
\n
• Maintain detailed documentation of all client interactions, treatment plans, and progress notes in our secure electronic health record (EHR) system.

• Engage in ongoing professional development by participating in case conferences and training sessions to enhance clinical skills and stay updated on best practices.

This role focuses primarily on supporting clients in a hospice environment, where you will work closely with terminally ill individuals and their families. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.
