Key Responsibilities\n

• Perform virtual comprehensive psychosocial assessments to identify clients' unique strengths and needs within the hospice context.

• Deliver evidence-based therapy and counseling services tailored to the individual's situation, whether in one-on-one or group settings.

• Collaborate with interdisciplinary teams to coordinate comprehensive, integrated care that addresses clients' mental health, social, and emotional needs.

• Advocate for clients' rights, ensuring they have access to necessary community resources and support services, enabling them to navigate complex care challenges.

• Document client interactions thoroughly, noting treatment progress and outcomes in electronic health records to maintain accurate and compliant documentation.

• Engage in regular team meetings, case conferences, and continuous education initiatives aimed at improving your clinical competencies and knowledge base.

Patient Population / Client Focus\n

You will primarily work with clients in hospice care, supporting them through one of the most challenging times in their lives. This may include helping individuals manage grief, depression, anxiety, and issues related to end-of-life decision-making. Your support will extend to family members, providing them with counseling to cope with their loved ones' illnesses and transitions.

Qualifications\n

• Master's degree in Social Work from an accredited institution.
\n
• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington state.

• A minimum of two (2) years of post-licensure experience providing clinical social work services, particularly in mental health or hospice settings.

• Strong competency in conducting diagnostic assessments and developing effective treatment plans based on individualized needs.
\n
• Exceptional interpersonal and communication skills, with a demonstrated ability to connect with diverse populations and build trust quickly.

• A commitment to cultural competence, social justice, and the ethical practice of social work.

Preferred Experience\n

• Certifications in evidence-based therapeutic practices, such as Dialectical Behavior Therapy (DBT), Acceptance and Commitment Therapy (ACT), and other relevant interventions.

• Experience in collaborative care environments, particularly in hospice or palliative settings.

• Proficiency in using telehealth platforms and electronic health record systems.

Compensation and Benefits\n
