Work Environment\n

This position operates remotely, allowing for flexibility while maintaining a collaborative approach to patient care. Even though you will be working from home, you will be actively integrating within a multidisciplinary team using various technology platforms for patient interaction, charting, and case management. You will be expected to meet productivity targets for billable services, thereby contributing to both the department's and the organization’s goals.

Key Responsibilities\n

• Collaborate with primary care providers to recognize and effectively treat mental health and psychosocial issues within the clinic, contributing to a holistic approach to patient care.

• Conduct thorough patient intakes focusing on diagnostic and functional evaluations, and provide recommendations for individualized treatment plans.
\n
• Deliver brief, focused interventions tailored to patients in immediate need of mental health services, ensuring timely support.
\n
• Advise primary care providers on the appropriateness of patient referrals to specialty mental health facilities based on clinical assessments.
\n
• Initiate follow-up communications to monitor patient progress and make necessary adjustments to treatment protocols.
\n
• Develop and implement relapse prevention plans for patients, equipping them with strategies to maintain stable functioning.
\n
• Facilitate connections between patients and additional behavioral health care providers, both within the primary care setting and externally, with appropriate patient consent.
\n
• Maintain accurate and comprehensive documentation of patient progress and treatment plans in compliance with regulatory standards.
\n
• Engage in proactive liaison with primary care providers to ensure continual communication regarding patient needs and treatment outcomes.
\n
• Educate patients on self-management techniques and foster their full participation in their recovery and treatment outcomes.
\n
• Provide insights and consultation to clinic management regarding behavioral services and associated program evaluation metrics.
\n
• Assist patients in adhering to medical treatments initiated by primary care providers, offering coping strategies for medication management.
\n
• Uphold the Code of Ethical Conduct for Clinical Professionals at all times.
\n
• Assist with additional duties as assigned by management to support team objectives.

Qualifications\n

To be considered for this rewarding opportunity, candidates must possess the following:

• A Master’s Degree in Clinical Counseling from an accredited institution.

• Current and active LPC (Licensed Professional Counselor) License.
\n
• Extensive knowledge of mental health assessment, treatment planning, and the principles of therapeutic practices.

• Ability to apply diagnostic criteria from DSM-IV and implement effective behavioral therapy techniques.
\n
• Proven capability to assess patient needs and evaluate care based on best practices.
\n
• Strong communication skills for effective collaboration within an interdisciplinary team setting and for meaningful interactions with patients and families.

• Ability to maintain clinical records in a thorough and organized manner.

Preferred Experience\n

While not mandatory, the following will enhance your application:

• Proficiency in bilingual communication, specifically in English and Spanish.

• Experience in outpatient care settings and familiarity with Federally Qualified Health Center (FQHC) guidelines.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 – 105,000 per year, complemented by a comprehensive benefits package. This includes health insurance, a sign-on bonus, paid time off, and opportunities for continuing education and professional development.
