Physical Therapist

We are seeking a licensed and experienced Physical Therapist to provide outpatient rehabilitation services in an in-home setting for the older adult population. This includes working in private residences or independent living facilities. Key Responsibilities Assess, develop, and deliver physical therapy programs to support rehabilitation. Identify the need for referrals to other disciplines as required for patient care. Implement individualized therapy plans to restore, reinforce, and enhance physical performance. Educate and train patients and caregivers on essential skills to promote independence and productivity. Direct health-promotion activities to maintain overall well-being. Why Join Us? Qualifications Valid Physical Therapy license in the applicable state(s) or proof of eligibility. Master's degree in Physical Therapy (or a related field) or a Bachelor's degree in Physical Therapy. Degrees earned outside the U.S. must be evaluated for equivalency. Minimum of 5 years of progressive experience in Physical Therapy. Valid driver's license or reliable transportation for in-home patient visits. Ability to meet physical demands, including lifting 50 lbs and standing or walking for extended periods.
