The Role

As an Advanced Practice Provider (NP or PA) , you will play a critical role in delivering comprehensive survivorship care for patients navigating life after cancer treatment.

This is a telehealth-first position —all visits are conducted virtually—allowing you to combine flexibility with meaningful, high-touch patient care.

What You’ll Do

• Conduct virtual survivorship visits and develop individualized, patient-centered care plans
• Manage long-term and late effects of cancer treatment , including chronic conditions
• Coordinate evidence-based surveillance and recurrence monitoring
• Provide education, counseling, and psychosocial support via telehealth
• Collaborate with a multidisciplinary care team (oncology, behavioral health, care navigation)
• Help design and refine clinical workflows, SOPs, and care protocols
• Contribute to quality improvement, patient experience, and program scalability

Who We’re Looking For

• Nurse Practitioner (NP) or Physician Assistant (PA) with active MO license (or ability to obtain a MO state licenses)
• Strong clinical judgment and comfort delivering care in a virtual environment
• Excellent communication skills with a patient-first, empathetic approach
• Ability to thrive in a fast-paced, evolving startup environment
• Highly organized with strong attention to detail
