This role is primarily remote, providing you with the flexibility to work from home while supporting clients from our hospice center in Danbury, Connecticut. You will collaborate with an interdisciplinary team, participating in virtual meetings and consultations to discuss patient care strategies and outcomes. Key Responsibilities\n

• Provide direct clinical services to clients coping with symptoms of major mental illness and severe emotional disorders, both in virtual settings and through home visits when necessary.

• Conduct thorough assessments and screenings to tailor integrated care plans that address the unique needs of each client.

• Facilitate individual, group, and family therapy sessions, ensuring that treatment aligns with clients' presenting problems and expressed goals.

• Engage in crisis intervention as required, providing immediate support and developing safety plans in collaboration with clients.

• Work collaboratively within an interdisciplinary team, sharing insights and strategies to enhance overall client care and outcomes.

• Ensure compliance with ethical guidelines and organizational standards during all interactions and documentation.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a related field is mandatory.

• Current LCSW licensure in Connecticut is required.

• A minimum of 2 years of clinical experience in a mental health setting, preferably with hospice or palliative care populations.
