Responsibilities: Provide direct patient care to residents, including administration of medications, wound care, and vital sign monitoring Develop and implement individualized care plans in collaboration with the interdisciplinary team Perform routine tasks such as bathing, dressing, and grooming residents Observe and report any changes in residents' conditions to the registered nurse or other healthcare professionals Maintain accurate and detailed records of patient care and treatment plans Participate in ongoing education and training to stay current with best practices and industry developments Requirements: Current LPN licensure in the State of Minnesota High school diploma or equivalent required; associate's degree or higher preferred Ability to lift, transfer, and reposition patients as needed Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment #J-18808-Ljbffr
