Key Responsibilities Evaluate assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction. Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities. Complete thorough medical record documentation in a clear, concise, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines. Ensure all care is provided under signed, valid physician orders and within insurance approvals as required for patient care. Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations. Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing. Required Qualifications Active and unrestricted Physical Therapist license in the state of New Hampshire. Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans with appropriate discharge goals. Proficiency in medical record documentation in compliance with state, federal, and payer guidelines. Commitment to patient and employee safety standards, including safe patient handling and infection control practices. Adherence to discipline-specific professional code of conduct and ethical standards. Preferred Experience / Skills Prior experience in an outpatient physical therapy setting. Experience supervising or mentoring physical therapy students. Active involvement in continuing education and professional development activities.
