Work Environment\n

This role allows you the flexibility of a remote work setting, while you will also interact with clients directly in their homes or senior living facilities. Our standard working hours are Monday to Friday from 8:00 AM to 4:30 PM, but you may occasionally need to adjust your hours to accommodate clients’ needs, including evenings or weekends. As an LCSW, you will maintain on-call availability to respond to urgent situations outside of regular hours, ensuring continuous care for our clients.

• Conduct comprehensive assessments of seniors, considering emotional, mental, and social factors to develop individualized care plans.

• Collaborate with clients and their families to establish personalized goals and navigation strategies for complex life transitions.

• Provide emotional support and counseling that enables seniors to effectively cope with challenges, including health issues and loss.

• Advocate for clients' rights and facilitate access to vital community resources, connecting them with services that promote their well-being.

• Work within an interdisciplinary team, coordinating with nurses, caregivers, and healthcare professionals to enhance client care.

• Monitor client progress through regular visits, ensuring the environment is safe and suitable for their needs while addressing any concerns that arise.

• Facilitate family meetings to mediate conflicts involving the care of clients, improving communication among involved parties.

• Educate clients and their families about local resources, support groups, and programs that aid in social engagement and quality of life.

• Maintain accurate client records, assessing needs and providing documentation of care plans and services rendered in compliance with regulatory standards.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Valid LCSW licensure in Texas.

• A minimum of two years of experience in social work, ideally within community services or a geriatric setting.

Compensation and Benefits\n

The annual salary for this position ranges from USD 60,000 to 100,000, commensurate with experience.
