We are hiring a Weekend Physical Therapist (PT) for a Home Health position, providing skilled care to patients in their homes in the Lansing area. Responsibilities Deliver skilled, patient-centered care in a home health setting, collaborating with an interdisciplinary team that truly supports you. Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence. Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home. Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation. Work independently, but never alone. Travel to patient homes within an assigned territory. Qualifications Active Physical Therapist, PT license in the state you work. One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care. Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
