Position Summary:
At Harvest Home, Residential Care Managers support residents throughout their journey from the moment they enter our Residential Program to their transition into housing and beyond, as part of our Alumnae Program. Care Managers work closely with each resident as case managers to develop a personalized care plan that reflects her unique needs, goals, and experiences. Daily, Care Managersmeet with residents, connect them to vital resources, and ensure that each residents care plan remains cohesive and supportive throughout our organization, from entry to graduation and beyond.

Our Care Managers are a vital part of our continuum of care to minimize relational transitions for residents as they move through our two-step residential program model. Step 1 focuses on stabilization, Step 2 focuses on preparation for birth and motherhood, and then in our Alumnae Program that aims to support in maintaining stability. Care Managers also provide case management and services for several families, which include expectant mothers with a child up to age 5.

Position Schedule:

Full-time Monday-Friday, business hours (40 hrs. per week) and occasional other days/times/hours as needed this includes role responsibilities as well as staff meetings, trainings and events. The position works at the Venice and Pico Homes and around LA County for home visits with our Alum. Flexibility needed to support one weekend shift per month for Alumnae-based activities (minimum of 3 times per year).

Responsibilities:

  • Provide individual care coordination for women receiving services and support from Harvest Home who are pregnant, recently postpartum, graduates (new mothers who have recently transitioned into housing), and their children (newborns to age 5 for those in our Harvest Home Family Program).
  • Meet with residents 1:1 to set individualized goals, develop a care plan, assist in overcoming barriers, and link residents to necessary resources including housing, employment, education, wraparound services, accessing public benefits, obtaining identification, etc.
  • Collaborate with other Care Managers to lead weekly group Care Coordination sessions to discuss residents and alumnae with the rest of the Care Continuum team (residential life staff, therapists, and Residential Leadership) regarding resident strengths, needs, and standing in the program to ensure the residents optimal functioning in our residential program including upholding household requirements, establishing a system of accountability, and follow through on areas of change, modification, and/or engagement.
  • Facilitate Critical Time Intervention case management model for those within their first-year post-graduation from Harvest Home, which includes home visits.
  • Listen attentively to resident concerns and suggestions and address them fairly and professionally, coordinate with immediate supervisor in working towards solutions.
  • Initiate, institute, and keep residents accountable for improvement plans where necessary/appropriate.
  • Facilitate linkage to mental health, substance abuse, and/or medical services, and provide transportation as needed.
  • Documentation using databases and systems to input, maintain, and organize services given and needed in a timely, organized, detailed, and accurate manner.
  • Advocate on residents and alumnae's behalf for support services in decision-making and self-advocacy.
  • Provide and/or coordinate crisis intervention assistance that aligns with agency approach to trauma-informed care.
  • Work with Residential Leadership to coordinate quarterly resources and class sessions for program graduates.
  • Provide financial assistance, resources, and support with budgeting, financial growth and collecting program fees, etc.
  • Promote personal growth, independence, responsibility, and accountability among the residents in the housing program and inspire them to reach their full potential.
  • Collaborate with the Residential Life Team on the provision of classes, workshops, groups, and events.

Community Network and Resources

  • Develop and maintain close working relationships with local service providers, outreach workers, case managers, and collaborative agencies.
  • Engage with external organizations to keep abreast of new, changing, and/or additional opportunities to ensure case management services and resources are appropriate, relevant, up-to-date, and applied to resident and program needs.
  • Attend community meetings to stay up to date with current resources, information, and community awareness within maternity/transitional home network, housing resources, employment resources, etc.

  • Ability to work well independently and collaboratively as a multi-disciplinary service team member.
  • Skilled in Trauma-Informed Care, Resident-Centered, Solution-Focused, and an empowering strengths-based approach.
  • Exhibits a high level of self-accountability and ethical standards.
  • Resourceful, motivated self-starter with the ability to manage time well and work autonomously; to manage multiple tasks and priorities, including proficiency in balancing direct service responsibilities with program, documentation, and organization responsibilities.
  • Excellent communication and critical thinking skills, solution-focused approaches, and crisis intervention skills with ability to exercise and model good judgment.
  • Strong relational skills with the ability to build rapport and trust quickly.
  • Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) and Project Management tools (Asana, Outlook calendar) required.
  • Ability to set, model, and uphold appropriate personal and professional boundaries and relationships with staff, residents, volunteers, and community providers.

Education and Experience

  • BSW with 2+ years relevant experience preferred; MSW; or other equivalent experience considered.
  • 2+ years case management experience with a similar population in a relevant (residential) setting, preferred.
  • Experience working with diverse, high-risk populations.
  • Strong understanding of and experience with the complexity of poverty and womens homelessness, mental illness, trauma, substance abuse, health issues, permanent supportive housing model and evidence-based practices.
  • Preference given to candidates with human services experience in LA County and with knowledge of local agencies and organizations.
  • Mature Christian faith, strong discernment, and model of servant leadership.

Other Requirements

  • Insured and reliable personal transportation, valid California drivers license, and clear driving record.
  • Smart phone that is compatible with work-related apps.
  • Comprehensive employment and criminal background check.
  • Physical requirements:
    • Job frequently requires sitting for extended periods of time
    • Must be able to lift 40 lbs.
