Key Responsibilities\n

As a Licensed Clinical Social Worker at Care Free Healthcare Center, your daily responsibilities will include:

• Implementing various evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored specifically to the needs of hospice patients.

• Maintaining accurate and timely documentation of clinical records in compliance with HIPAA regulations and organizational standards.

• Collaborating with a multidisciplinary care team consisting of physicians, nurses, and chaplains to provide holistic care that addresses the physical, emotional, and spiritual needs of patients.

• Facilitating family meetings to educate patients and their families about support resources, treatment options, and coping strategies during the end-of-life journey.

• Participating in case consultations and peer review meetings to share insights and improve the quality of care provided to patients.

Patient Population / Client Focus\n

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

Compensation and Benefits\n
