This full-time role focuses on improving the lives of seniors in the Quantico, Maryland community and surrounding areas. As a Licensed Certified Social Worker (LCSW-C), you will provide vital emotional and mental health support, develop personalized care plans, and coordinate services tailored to the unique needs of our clients.

Work Environment\n

This position is primarily remote, with occasional visits to clients’ homes or facilities as needed. The role is built around a full-time schedule, typically Monday through Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on specific client needs. Flexibility is essential to accommodate client visits and ensure continuous support, as well as being on-call for urgent situations that may arise unexpectedly.

Key Responsibilities\n

Your day-to-day responsibilities will include:

• Conducting comprehensive assessments of seniors' psychosocial needs, considering emotional, cognitive, and social factors to better understand individual circumstances.

• Developing and implementing personalized care plans in collaboration with clients and their families, setting achievable goals that enhance their quality of life.
\n
• Providing counseling and emotional support to seniors, assisting them in navigating significant life transitions or challenges, such as loss or health crises.
\n
• Advocating for clients by helping them access community resources, such as financial assistance programs, healthcare services, and social groups.
\n
• Working collaboratively with an interdisciplinary team, including healthcare providers, caregivers, and service coordinators to create a holistic care environment.
\n
• Monitoring and documenting client progress through regular visits, maintaining accurate and up-to-date records within our electronic health record system.
\n
• Educating clients and their families about available resources and programs to improve social engagement and well-being.
\n
• Facilitating family meetings and mediating conflicts to ensure clear communication and support among involved parties.

Patient Population / Client Focus\n

The primary focus of this role is on seniors aged 60 and above, who may face a variety of challenges related to aging, including mental health issues, chronic illnesses, or social isolation. You will provide empathetic and respectful care to diverse populations, recognizing the unique cultural, economic, and social factors affecting each individual. Your work will help clients access necessary support while fostering their independence and social well-being.

Qualifications\n

To thrive in this role, candidates must possess the following qualifications:

• A Master's degree in Social Work from an accredited educational institution.

• Current LCSW-C licensure in the state of Maryland.
\n
• At least two years of relevant experience in a social work capacity, ideally within geriatric settings or community services.

• Deep understanding of the aging process and related psychosocial issues.

• Exceptional interpersonal and communication skills, fostering positive rapport with clients and families.

• Strong assessment, counseling, and problem-solving abilities, tailored to meet the specific needs of seniors.

• Competence in managing electronic health record (EHR) tools and related software applications.

Preferred Experience\n
