What You’ll Do We're excited to add a Senior Case Manager to our Transitional Housing team. This role provides intensive, strength-based, trauma-informed services to families experiencing homelessness who are participating in a 12-month transitional housing program. Reporting to the Program Manager, the Senior Case Manager partners with families to address the complex barriers that contribute to housing instability and family crisis. Through assessment, intervention, counseling, skill-building, advocacy, and coordination of services, the Senior Case Manager helps families strengthen their functioning, increase self-sufficiency, and prepare for successful independent living at program exit. This role goes beyond traditional case management and requires a professional who can build therapeutic relationships, facilitate meaningful behavioral change, support family wellness, and develop individualized interventions that lead to long-term housing stability. What Your Day Looks Like Family Assessment & Intervention Conduct comprehensive biopsychosocial, housing, and family functioning assessments upon program entry. Develop individualized Family Stability Plans with measurable goals related to housing, income, health, mental health, parenting, education, and self-sufficiency. Provide ongoing therapeutic and solution-focused interventions to support families in overcoming barriers to permanent housing. Utilize trauma-informed, strength-based, harm-reduction, motivational interviewing, and family-centered approaches in service delivery. Assist families in identifying and addressing challenges related to mental health, substance use, domestic violence, parenting stress, trauma, and social isolation. Monitor progress and adjust intervention plans based on family needs and outcomes. Housing Stability & Independent Living Preparation Support families in developing the skills, knowledge, and confidence necessary to maintain permanent housing after program completion. Facilitate education and coaching related to budgeting, financial literacy, tenant rights and responsibilities, conflict resolution, time management, employment readiness, and household management. Partner with families to establish sustainable income and employment plans. Assist participants in building natural supports and community connections that will remain in place after program exit. Create individualized transition plans that promote successful movement from transitional housing to permanent housing. Family Wellness & Child Development Support healthy family relationships and positive parenting practices. Connect children and youth to educational, developmental, behavioral health, and enrichment resources. Work collaboratively with caregivers to strengthen family resilience and promote child wellbeing. Identify and address factors that may impact family functioning and long-term stability. Crisis Intervention & Advocacy Provide crisis intervention, de-escalation, safety planning, and stabilization services as needed. Advocate for families with landlords, schools, healthcare providers, behavioral health providers, public benefits agencies, and other community systems. Support families in navigating complex service systems and accessing critical resources. Participate in multidisciplinary case consultations and program meetings. Coordinate services with internal teams and external service providers to ensure integrated care. Maintain accurate and timely documentation in electronic databases and participant files. Track outcomes, complete reporting requirements, and utilize data to inform service planning and program improvement. Represent Hamilton Families at community meetings and partnership collaborations. What Makes You Successful in This Role Demonstrated ability to build trust and engagement with families experiencing homelessness, trauma, and housing instability. Strong clinical assessment, intervention, and crisis management skills. Understanding family systems, child development, trauma, and the impacts of homelessness on family functioning. Ability to balance supportive relationships with accountability and goal achievement. Excellent documentation, organization, and care coordination skills. Ability to work collaboratively with diverse populations and multidisciplinary teams. Commitment to equity, cultural humility, and participant-centered service delivery. Strong problem-solving and advocacy skills. What You Bring Required Qualifications Master's degree in social work (MSW), Marriage and Family Therapy (MFT), Counseling, Psychology, or a closely related behavioral health field. Minimum 2 years of experience working with vulnerable families, individuals experiencing homelessness, or similar populations. Experience providing direct interventions, counseling, family support, or behavioral health services. Knowledge of trauma-informed care, crisis intervention, motivational interviewing, and strength-based practices. Experience developing service plans and facilitating behavior change with participants. Preferred Qualifications Licensed or licensed-eligible (ASW, AMFT, APCC, LCSW, LMFT, LPCC, or equivalent). Experience in transitional housing, supportive housing, family homelessness, child welfare, behavioral health, or community mental health settings. Familiarity with Bay Area housing and social service systems. Bilingual abilities, especially Spanish. Experience with Salesforce, HMIS, or other case management databases.
