The People Concern seeks a devoted HSSP Substance Abuse Case Manager to support residents in permanent supportive housing across SPA 7 & 8. You will serve as a primary case manager, guiding members toward recovery, coordinating inpatient or intensive treatment as needed, and ensuring high-quality, wrap-around services. You will collaborate with licensed clinicians and the on-site team, maintain documentation, and promote wellness, coping, and housing stability. #J-18808-Ljbffr The People Concern
