Work Environment

This is an onsite role wherein the Nurse Practitioner will primarily provide care in the homes of patients, allowing for a unique and personal approach to healthcare. Collaborating closely with a team of dedicated healthcare providers, including physicians, nurses, and therapists, you will work together to ensure seamless and comprehensive care delivery.

Key Responsibilities

• Conduct thorough patient assessments through physical exams and health evaluations to determine the needs of each individual.
• Diagnose a wide range of medical conditions, both acute and chronic, utilizing evidence-based practices and collaborative input from the care team.
• Develop and execute personalized treatment plans tailored to the specific health requirements and preferences of patients, including medication management.
• Regularly monitor patient progress and make necessary adjustments to treatment plans based on ongoing assessments and feedback.
• Educate patients and their families on various health issues, treatment options, and wellness practices to empower them to take an active role in their health journey.
• Maintain precise and compliant patient records in accordance with HIPAA regulations to ensure confidentiality and adherence to legal standards.
• Coordinate care with multidisciplinary teams, fostering communication and collaboration to provide optimal health outcomes.

Patient Population / Client Focus

The ideal candidate will focus primarily on adult and geriatric patients in their homes, emphasizing chronic disease management, preventive care, and rehabilitation support. This role requires sensitivity to diverse health issues faced by these populations, including mobility challenges, cognitive impairments, and socio-economic factors impacting healthcare access. You will play an essential role in improving their quality of life.

Qualifications

• Active Nurse Practitioner (NP) license in the state of Florida.
• Completion of a Master of Science in Nursing (MSN) with a focus on Nurse Practitioner studies.
• Board certification in a relevant specialty area, such as Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Psychiatric-Mental Health Nurse Practitioner (PMHNP).
• Active DEA license and NPI number required.
• Florida Board of Pharmacy license.

Preferred Experience

While two years of experience in an outpatient, hospital, or primary care setting is required, candidates with extensive experience in home care or community health will be highly favored. Familiarity with electronic health records (EHR) is also advantageous.

Compensation and Benefits
