Work Environment

This position is based in Tallahassee, Florida, and involves providing care in the patient's home environment. As a member of our on-site care team, you will be responsible for creating a warm and welcoming atmosphere for patients and their families. Our home care facility allows for personalized interactions and the ability to assess and address health needs in a familiar setting, which can significantly enhance patient comfort and create opportunities for detailed health assessments.

Key Responsibilities

• Conduct comprehensive health assessments including obtaining thorough medical histories and performing physical examinations on pediatric patients.
• Develop individualized care plans that prioritize preventive care, immunizations, and ongoing treatment protocols.
• Perform a variety of therapeutic procedures, including but not limited to immunizations, wound care, and other necessary interventions aligned with best practices.
• Order and interpret diagnostic tests and imaging to evaluate the effectiveness of treatments and to make informed decisions regarding patient care.
• Chart and document all patient interactions accurately and diligently, following organizational policies and ensuring all records comply with legal and regulatory standards.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care coordination, managing patient referrals as necessary.
• Participate in quality improvement initiatives and utilize feedback to enhance clinical practices.
• Engage with families by providing health education and resources to empower them in supporting their children's health and development.
• Facilitate and attend team meetings to discuss patient care and service delivery improvements.
• Adapt to fluctuations in patient flow, including evenings and weekends as needed, to meet community healthcare demands.

Patient Population / Client Focus

This position primarily focuses on infants, children, and adolescents, addressing a wide range of health concerns and developmental needs. You will interact closely with patients and their families, providing them not only with medical care but also guidance on health education, nutrition, and preventative strategies tailored to pediatric populations.

Qualifications

• A valid Nurse Practitioner degree from an accredited institution, with a specialization in pediatrics.
• Active Pediatric Nurse Practitioner (PNP) certification.
• Proven clinical experience in pediatrics, demonstrating the ability to respond effectively to emergencies and critical situations.
• Strong documentation and charting skills, with the capability to maintain accurate patient records in compliance with regulations.
• Excellent interpersonal and communication skills, capable of building trust with young patients and their families.
• Ability to work collaboratively with a multi-disciplinary team in a dynamic healthcare setting.

Preferred Experience

• Experience in a home care environment is highly desirable.
• Bilingual proficiency in English and Spanish is preferred, enhancing communication with a diverse patient population.
• Demonstrated commitment to continuing education and staying abreast of advancements in pediatric health care practices.

Compensation and Benefits

Pediatric Plus Health Group offers a competitive salary ranging from USD 131,000 to 150,000 annually , commensurate with experience and qualifications.
