Lifeline Family Care Center is looking for a dedicated and experienced Family Nurse Practitioner (FNP) to join our dynamic team in Tallahassee, Florida. The FNP will work onsite to deliver comprehensive primary care services without the pressures of on-call requirements or weekend shifts.

Work Environment

The role entails practicing onsite with established hours of operation from 8:00 a.m. to 5:00 p.m. on Mondays, Wednesdays, and Fridays, allowing for a balanced work-life experience.

Key Responsibilities

• Conduct thorough health assessments, including physical evaluations and comprehensive medical histories.
• Perform health risk assessments on-site, identifying patient health concerns and developing tailored intervention strategies.
• Provide effective patient education on preventive health strategies and lifestyle modifications to promote wellness.
• Assist individuals in managing chronic health conditions by offering personalized treatment plans and ongoing monitoring.
• Coordinate care with the medical staff within the Lifeline Family Care Center to ensure consistent quality care.
• Develop collaborative working relationships with colleagues, fostering a healthcare team that prioritizes effective patient outcomes.
• Engage in health promotion initiatives through community wellness events and education fairs.

Patient Population / Client Focus

The FNP will primarily serve patients of all ages who seek comprehensive and personalized primary care. Our clientele comprises community members who prioritize their health through preventative care models, emphasizing health management and educating patients on their conditions to help them lead healthier lives.

Qualifications

• A Master’s degree in Nursing with a specialization in Family Nurse Practitioner.
• Current and unrestricted nurse practitioner licensure in Florida.
• A minimum of 2 years of experience in a primary care or family practice setting.
• Proficiency in using Electronic Medical Records (EMR) systems.
• Strong belief in making a positive difference in patient lives through quality care, effective communication, and teamwork.
• A willingness to actively participate in community wellness initiatives.

Preferred Experience

• Familiarity with concierge medicine practices and the ability to navigate patient-centered healthcare models.
• Experience in delivering care with a focus on prevention and wellness.

Compensation and Benefits
