Key Responsibilities

• Conduct thorough assessments and accurately diagnose both acute and chronic medical conditions among diverse patient populations.
• Develop, implement, and evaluate individualized treatment plans that are responsive to patients’ specific health needs, while leveraging diagnostic tests as necessary.
• Prescribe medication and initiate other therapeutic interventions, monitoring patients' responses to treatment and adjusting as needed.
• Educate patients and their families on disease prevention, health promotion strategies, and lifestyle modifications through one-on-one and group sessions.
• Collaborate with a multidisciplinary team of physicians, nurses, and specialists to ensure comprehensive coordination of care and optimize patient outcomes.
• Maintain meticulous and accurate medical records adhering to industry standards and regulations while ensuring patient confidentiality.
• Engage in quality improvement initiatives aimed at enhancing patient care processes and outcomes.
• Stay informed about the latest developments in healthcare practices and regulatory standards through continuous education.

Patient Population / Client Focus

Your primary focus will be on serving a diverse outpatient population, addressing a wide range of medical needs across various demographics. This includes managing chronic illnesses, preventive care, acute conditions, and health education to empower patients in their healthcare journeys. You will work closely with patients from diverse backgrounds, emphasizing culturally competent care tailored to their unique circumstances.

Qualifications

• Current certification as a Nurse Practitioner.
• Active nursing license in the state of Florida.
• A Bachelor's degree in nursing or a related field; a Master's degree in nursing from an accredited program is preferred.
• At least three years of clinical experience in outpatient or a relevant specialty setting.
• Proven ability to communicate effectively with patients, families, and colleagues, demonstrating excellent interpersonal skills.
• Strong critical thinking, analytical, and problem-solving abilities.

Preferred Experience

Experience in family medicine, internal medicine, or urgent care is highly desirable.
