Clinical Care Manager RN

VitalCaring is seeking a Clinical Care Manager RN to lead and coordinate home health services for a diverse caseload. In this role, you’ll develop care plans, oversee field clinicians, and ensure clinically excellent, compassionate care in the home. You will monitor outcomes, support compliance with Medicare and regulatory standards, and partner closely with physicians and an interdisciplinary team.
