Key Responsibilities\n

• Conduct comprehensive assessments to identify the mental health needs of your clients, tailoring treatment plans to address their specific situations.

• Maintain open lines of communication with clients, families, guardians, and other healthcare providers about clients' progress and any adjustments needed in treatment.

• Regularly provide supervisors with scheduling updates for appointments and home visits, ensuring all activities are documented and up to date.
\n
• Coordinate home visits for in-depth observations, chart reviews, and data collection, guaranteeing that care plans are effectively implemented.

• Participate actively in Interdisciplinary Team Meetings to discuss patient status and collaborative care strategies.
\n
• Maintain detailed records and progress notes on a monthly and quarterly basis to ensure compliance and continuity of care.
\n
• Liaise with the public and other mental health community stakeholders to advance development goals and extend our outreach.
\n
• Organize and lead training sessions for staff and team members to ensure competence in implementing our program plans and documentation standards.
\n
• Provide follow-up interviews and ongoing support to help clients achieve their treatment goals.
\n
• Contribute to the development of procedures and training materials that enhance programming for individuals receiving care.

Patient Population / Client Focus\n

In this role, you will work with a diverse client base, including adults and youth from various backgrounds. Your work will directly impact individuals facing mental health challenges such as anxiety, depression, trauma, and substance misuse. You will help clients navigate their journeys towards recovery, ensuring inclusivity and tailored approaches for every individual.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted Professional Counselor license in the state of Colorado is essential.

• A minimum of 2 years of counseling experience, ideally within a clinical setting, is required.

• Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse counseling is highly preferred.
