Description

• Conduct thorough evaluations in inpatient and outpatient settings, crafted into individualized treatment regimens
• Implement targeted physical therapy strategies to enhance mobility, strength, balance, and overall functionality
• Work collaboratively with a diverse team of healthcare professionals including nurses, physicians, and case managers to maximize patient outcomes
• Educate patients and their families about treatment objectives and home exercise routines and help them prepare for discharge
• Keep meticulous records of patient evaluations, treatments, and advancements to adhere to regulatory and facility standards
• Engage actively in care team huddles and interdisciplinary meetings to ensure seamless patient care

Education

• Earned a Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution

Certifications

• A valid state license as a Physical Therapist
• CPR certification is preferred
• Additional certifications (e.g., neurological, geriatric specialties) are advantageous but not mandatory

Skills

• Prior experience in a clinical setting is highly preferred
• Expertise in treating patients with varying complexities in medical conditions and mobility
• Exceptional communication and teamwork abilities
• Proficient with digital documentation and clinical record-keeping systems
