RN Care Manager - Pineville Clinical Care Management PRN Days

Job Summary Facilitates lifelong, proactive partnerships with patients to enhance and personalize management of health-related needs. The Care Manager assesses needs, plans, coordinates, and evaluates services for patients to equip them and their families to easily access resources and adopt healthy lifestyles that increase their ability to remain in the most appropriate care setting. Essential Functions Utilizes RN process to focus healthcare team activities on optimal outcomes, resource utilization, clinical expertise, and improvement strategies. Interacts with patients, professionals, and the community to maintain continuity of care, coordinate services, and document care plans across multiple settings. Conducts comprehensive “all-system” needs assessments for identified patients and matches appropriate care-related services to disease management and health maintenance goals. Develops and maintains accurate case records and ensures complete, accurate documentation across computer databases and systems. Maintains knowledge of reimbursement criteria and necessary documentation for Medicaid, Medicare, and Managed Care, including billing standards and regulations. Demonstrates leadership in nursing practice, evaluating own practice against professional standards, regulations, and relevant state laws. Qualifications Bachelor of Science in Nursing (BSN) required; Master’s preferred. Current RN license or temporary license in the state of residence, or compliance with National License Compact requirements. Two years of health‑care experience, including case management or discharge planning in acute care, home care, long‑term care, physician office, or managed care settings. Professional certification in case management within three years of hire, as per departmental protocol. Clinical competence in disease management and case‑management principles. Excellent interpersonal communication, negotiation, problem‑solving, organizational, and time‑management skills. Ability to work independently and as part of a multidisciplinary care team. BLS certification required. Physical Requirements Ability to walk, stand, lift, reach, bend, and stoop. Must lift a minimum of 35 pounds at shoulder height. Must travel or drive between various locations. Frequent verbal and written communication in English; intact sight and hearing, finger dexterity, critical thinking, and concentration. Ability to respond quickly to changing assignments. Tolerance of occasional intermittent noise and exposure to dust, fumes, and chemicals. Benefits Base compensation within the pay range of $38.20–$57.30, based on qualifications and experience. Premium pay for shift, on‑call, and other compensations.
